e-Medsys Patient Access Portal Patient Forms

Lets Your Patients Fill Out Forms From Home

Practices can create forms such as “Online Patient Form” via e-Medsys EHR, and make them available to patients to complete via the Patient Access portal.

Patients will see the practice’s forms under the “Patient Forms” section along with a note from the practice. While completing in the form, the patient can send messages or ask questions regarding the form via the secure messaging function (see the “Secure Messaging” tab).

When the patient submits the completed form, it will appear in the review section of the Doctor’s Home Page in the e-Medsys EHR for review, and it also becomes part of the patient’s record the e-Medsys EHR, eliminating the need for patients to complete paper forms on arrival (“The Clipboard”) which then must be entered into the patients record manually by office staff.
